At approximately 1:26 PM today, Crowder employees began reporting an email as phishing with subject: IMPORTANT TAX RETURN DOCUMENT AVAILABLE from mail@onlineformretrieval.com.

After reviewing the email and speaking with HR, it was determined that this email is legitimate, containing your 1095C.  Any emails that were quarantined from Crowder inboxes have been restored.  If you previously reported the email as phishing using the "Phish Alert Button", the original email may be found in your "Deleted Items" folder in Outlook.

This email may also have gone to your personal email inbox (i.e. yahoo, gmail, etc.) if that was listed as your primary email address with our health insurer.
